Medication Therapy Management

Medication Therapy Management is a form of pharmacist intervention to prevent or resolve drug therapy. MTM was defined by eleven national pharmacy organizations in July 2004. MTM services can involve a broad range of professional activities, including developing a medication treatment plan (Download Medication Management Form [pdf]), identifying and resolving drug-related problems, and providing patient education and training. MTM services should be patient-specific and provide face-to-face interaction when possible. Pharmacists or other qualified providers should be the only professionals offering this therapy.

University Pharmacy has trained pharmacists who perform these therapy sessions. Many of these sessions are covered by insurance providers.
